Located at 2325 N. Nebraska Ave. Helps maintain the independent lifestyle and dignity of older adults in this community. It works to develop and maintain a network of coordinated, community-based services and activities. Director is Lori Byers, 362-7626
CARE MANAGEMENT SERVICES
The program’s care manager is trained to help older people through difficult situations. By providing a professional assessment of need and through available resources, those involved can make wise decisions.
Following the assessment, the care manager will: explain available services and all possible options, make arrangements and help plan how to make best use of community resources, help make arrangements to get services started, continue to work with the older person to make sure the plan is working.
Fees are based on the client’s income. Call 362-7626.
MEDICARE & INSURANCE INFORMATION
Persons with questions about Medicare, Medicaid and supplemental insurance can receive help from the trained Nebraska Health Counseling and Assistance Program project. Call 362-7626 for an appointment with a counselor.
SENIOR MEALS
Served in a group setting in York and McCool on Wednesdays and Fridays. Meal reservations need to be made 24 hours ahead. People 60 or older and their spouses make a donation for the meal. Younger diners pay a set fee. For more information or to reserve a meal call: 724-2525 (McCool) or 362-7626 (York).
IN-HOME SERVICES
Assists in arranging in-home services for older persons who may need help with: cleaning, cooking, laundry, shopping and errands, yard work, minor home repairs, snow removal, personal care tasks, respite care.
Match-ups are made with the independent contractors in the community, or referrals made to appropriate agencies that provide the needed service.
The costs for these services are paid by the older adult. Low income persons may qualify for assistance in paying for needed services.

TRANSPORTATION
Transportation services are provided by the County Bus and Busy Wheels.
